Live Animals and Plants
Original of Veterinary Health Certificate. Photocopy of the passport of the pet's owner. Vaccination Certificate. Import License. Import License. Notarized Health Certificate from Veterinarian. Quarantine IS NOT required for cats and dogs. Contact destination agent prior to forwarding.
